Please fill out the Application Form completely. Be sure all persons over 18

sign both Page 4 and Page 5. If there are more than 2 people over 18,

please print additional copies of Page 5 as needed. If any person in the

household (over 18) is unrelated, they must fill out a separate Application Form.

Please submit the completed Application, along with the appropriate

Income & Asset Statements (click below), to DeerCreek Village Apartments.

Your Application paperwork will be valid for 90 days.

Each person in your household who is 18 years of age or older is required to

complete and sign a separate form. Please check “YES” or “NO” to each

question, fill in the appropriate dollar amounts and balances where necessary,

print and sign your name at the bottom, and submit the form to

DeerCreek Village Apartments along with your Application.  “Thank you.”
